Using the new gift form
Want some more details on how to use the new form? Here are a few tips to help you:
Section 1: The basics: who you are, where you live
We need this information so that we can contact you if there is a problem with your gift and to ensure that charitable receipts are sent to the correct location.
You can help us tailor the annual campaign to meet the needs of specific demographics by filling out the language of preference and age-range questions.
If you are leaving the workplace (retiring, changing jobs or going on maternity leave), please check the box in section one and your local United Way/Centraide (which manages the GCWCC) will contact you.
Section 2: Distributing a gift
This is where you decide where you want your gift to go to work.
Once you have decided on your total gift, please write it in at the bottom of each applicable box. Divide your total by 26 if you are giving through payroll and indicate the total amount in the box at the far right.
Box A - United Way/Centraide
You can choose United Way/Centraide (either in your region or any other across the country — see the GCWCC calendar for a complete list). Depending on your local United Way/Centraide, you may be able to make a gift to specific impact areas, which will be listed on the form.
Box B - Healthpartners
You can choose to give to all 16 charities equally or you can specify which ones you want to support and the dollar amount you want to give for each.
Box C – Other Registered Canadian Charities
Write in the name, registration # and amount you wish to donate to any other registered Canadian charity. To find a list of registered #s, please visit the Canada Revenue Agency website (http://www.cra-arc.gc.ca).
Section 3: Total Gift
Add up all your donations from Boxes A, B and C to indicate your total gift.
Section 4: Leadership Gifts
In many regions, the names of Leadership Givers are included in a thank you document. If you give at this level, you can choose to not have your name included in the document in your region.
Please note: The level of leadership giving varies based on the region; for information for your area, please contact your local United Way/Centraide.
Section 5: How to make your gift
In this section you can select several options to make a gift.
If you are selecting the payroll deduction option, you can proceed to section 6.
If you are choosing to give by credit card, cash or cheque, please fill out the appropriate information and submit your form to a campaign volunteer.
Note for donors in the NCR: In this section you can find check boxes if you wish to receive more information regarding other giving options available through the GCWCC.
If you choose to give through gifts of stock, you can contact a representative directly at the number indicated on the form. If you would like more information regarding other giving methods, please check the appropriate box and a campaign representative will contact you once your gift has been processed.
Section 6: Payroll deduction
Note: Donors making a gift through payroll deduction must not detach section 6 of the form. It will be detached and forwarded to your payroll office by a campaign volunteer after it’s been entered into the system by your campaign treasurer.
Fill out this section to complete your payroll deduction. Please be sure to enter all the information so that your payroll office can start the deduction process.
Once you’ve completed the form, please give it to a campaign volunteer in your section. The volunteer will pass the form on to campaign treasurers in your department, who will enter the information and then tear off the payroll information section and forward to your payroll office.
The top portion of the form will be sent to your local United Way/Centraide so that information can be verified and in some cases entered into a database.
Please note: All information that you enter in section 6 will only be used by the payroll office in your department/agency. Once the information is entered into the system, your finance department will keep the hard copy on file. None of the information will be shared with anyone outside of your organization.
Note about numbering:
Each gift form has a unique number, located in the top corner and in the payroll deduction section on one side of the form. This number is required for data tracking through the online system, which means that forms cannot be photocopied or duplicated as the system won’t allow for multiple entries of the same number.
Notes about receipts:
The biggest change you’ll notice about the new gift form is that it’s a one-piece with a tear-away section for payroll deduction. Following significant consultation with donors, the vast majority indicated that they have not kept the receipt copy of the gift form in the past. This fact, along with donors’ requests for a more environmentally friendly campaign, have led to the creation of the one-part form.
As a donor, if you do wish to keep a record of your gift, you have three options:
1. If you give through payroll deduction, you will see the amount of your gift on your pay stubs.
2. If you give through cash, cheque or credit card, you will receive a charitable receipt after the end of the campaign that contains your donation information.
3. If at any time you have questions or concerns about your gift, contact your local United Way/Centraide, which will have a copy of your original gift form on file (not including the payroll section).
